The Organ trafficking ring was a secret operation conducted by corrupt guards of the Squid Games to harvest organs and sell them on the black market. It was led by the Masked Officer.
During the 33rd Squid Game, the operation was immediately put to a stop after it was revealed that they gave Byeong-gi information about the next games, which was against the rules, in exchange for Byeong-gi performing the organ procurement. All guards involved were killed by the Squid Game guards under orders of the Frontman and their bodies were hung alongside Byeong-gi to make an example out of them.
During the 37th Squid Game, the operation is shown to still be ongoing, now with a doctor hired from outside the Squid Games to perform the operations. The involved soldiers non-fatally shooting other eliminated participants to ensure that viable organs can be extracted from the victims, though Kang No-eul made things difficult by ensuring that the eliminated players were dead by shooting them again.
